With Flex, you can attach consent forms to patient's treatment plans based on procedure codes and/or categories. Coming soon: Automatic consent forms, stay tuned for more details!
In the meantime, this guide will show you how to link your Flex consent forms to your Open Dental procedure codes.
Access
- Log in as an Admin User↗️.
- On the homepage, in the navigation menu, click Settings.
- In Forms, go to Consent Forms:
Linking Forms
- To begin linking codes, select a consent form from the drop down menu.
- Use the link icon to open the setup window.
- Choose how to link forms:
- Codes: Open Dental treatment codes.

Categories: Open Dental treatment categories.
- ⚠️Note: If you are using Categories instead of Codes, ensure your Categories are set up to your preferences in Open Dental. Here is a helpful guide from the Open Dental support center to assist you: Procedure Codes & Categories↗️.
- In the bottom right, click Save Changes.
- To apply your changes, Save again in the lower right corner of Consent Forms Settings.
Once saved, when a patient signs the Consent version of the Flex Treatment Plan, or if you send them with the Present version, they will be prompted to sign all attached consent forms after signing the treatment plan.
💡Pro Tip: If you choose to link individual treatment codes, you do not need to select the matching category. Both options are available for your convenience, if you prefer to link multiple codes at once, use categories.
