How Do I Attach Teeth To A Consent Form?

You can attach any number of teeth/surfaces to your patient's consent form prior to having them sign or let the automated system do it for you! To set up the automated system, view this guide: How Do I Attach Consents To Treatment Plans?↗️

To manually add this information, begin by searching for your patient:

Next click on the 'consent' icon on the patient profile:

Use the '+' icon on any of the consent forms you are going to send and/or have the patient sign in house:

Once selected, Flex will load up and display options for teeth, surfaces, and a toggle for using the US or International systems. Simply click on the tooth/teeth and surfaces and choose the '+' icon to add them:

Once done, you will see your teeth added on the consent list, and these additions will also show on the consent form itself, and will be including on the .pdf that is sent to your practice management system after being signed by the patient:

💡Pro Tip: Save time and let Flex add these details for you! When treatment codes are linked, the system reads your schedule and your patient's saved treatment plan in Open Dental.
