The Flex Self Check-In Kiosk automatically prompts consent forms at check-in based on the procedure codes attached to the patient’s appointment. This guide covers how consent forms are triggered and what to expect throughout the process.
Set Up:
To ensure consent forms appear on the kiosk at check-in, three things are required:
- Consent forms must be linked to the appropriate procedure codes.

- Your office must be using Advanced Forms. The resources below will help you confirm which forms are enabled in your Flex settings and guide you on switching to Advanced Forms if needed:

Kiosk appointment status mapping must be configured in Flex Settings.

What to Expect:
When a patient checks in using the kiosk:

Patients due for an intake or update forms, are encouraged to complete those forms first.
Patients who only need to complete consent forms will not see an intake or update form as pictured in this example. -
Linked consent forms attached to the appointment’s procedure codes are presented.
- Once all forms are completed, patients are automatically directed back to the kiosk to complete check-in.
- Patients click Submit to complete check-in.
- Check-In is confirmed.
Notes:
- If a patient attempts to skip a form, the kiosk will direct them to your front desk team.
- Choosing Proceed allows the patient to check-in without completing forms.
