Sending your patients general forms or consents forms is a very simple process!
First you will want to search for your patient, or select them from the schedule to open their profile:
Once on the patient profile page, click on the 'consents' icon and this will load a list of all your office's consent forms:
Once selected, choose any number of consent forms from the list (when selected they will highlight in blue). You may also add any number of teeth per consent form by using the '+' icon. To let Flex add tooth numbers automatically, first link your treatment codes. Learn how to link codes in this guide: How Do I Link Treatment Codes to Consent Forms?↗️
Once you have selected all your consent forms, the final step is to choose the 'text or email' icons on the bottom of the page:
Please note: Your patient's must have a valid email and/or phone number for Flex to send to. In my example my patients does 'not' have a valid phone number, but has a valid email. Therefor I am only able to send the consent form via email until a valid phone number is entered into the patients account in the practice management software.
