Please keep in mind that your patient(s) must have a valid email/wireless phone number, otherwise, you will be prompted with an error message.
After logging into Flex’s Homepage, search for your patient or select your patient from the day’s scheduled patients list:
Once you select your patient their Profile Page will load. From this page, select the ‘Forms’, ‘Consent’, or ‘Post Op’ icon from the menu at the top left of your screen:
Flex will display an (!) over the icon when a patient is due for a form like so:
Each icon will open a list of your Forms, Consents, and/or Post Ops.
Select the form you would like to send to your patient from the list. You may choose one or multiple forms at once. Now that you have your form(s) selected, choose the method by which you would like to send the form(s) by selecting one of the following icons: email, text, or tablet.
When selecting a Form such as a New Patient Form (NPF) or a Health History Update (HHU) for a patient who has multiple family members due for the same form, you may select the (+) icon at the top to add family members. Any selected family members will have the same forms included when sent via email, text, or tablet.
