Sending forms individually can be time consuming and create extra follow-up. In Flex you can send forms to multiple family members at once helping to streamline the process, reduce admin work, and ensure that all required information is completed before the visit. In this guide you will learn how to send digital forms to multiple family members in a single message.
Steps to Send Family Forms Manually
- In Flex, access the ‘patient profile’.
- In the upper-left toolbar, click ‘forms’.
- In the web forms window, tap the plus (+) icon at the top to add other family members.
- Use the toggle switch to include forms for each additional family member.
- In the bottom right, click ‘Close’ to attach forms to the message.
- From the list, select which forms to send to your patient.
- At the bottom of the web forms window, choose how to send your forms.

Once sent, the link or tablet will grant access to the needed forms, saving time at check-in and improving the overall experience for both your team and your patients.
