How to Set Up Nested Forms in Flex

Nested forms are designed to let you embed a standalone form into an existing form you've already created in Flex. When imported into Open Dental, a nested form will be treated as a separate PDF. Any standalone form can be nested inside another form. 

 

In this Text Guide we will be using the example of nesting a Records Release in a New patient Form:

When nesting/adding any form into another from, be sure to create each form as its own stand alone form in the from builder. In this case we will need to have a New Patient Form and Records Release form created as their own standalone forms. 

 

Next, in the form editor open your New Patient Form and determine the best place to include a question like, "Would you like us to request your dental records from another provider's office?"

 

  • Update your ‘Reference’ to a user friendly phrase like ‘recordsrelease’
  • Add your responses/options of ‘Yes’ and ‘No’.

 

From the elements on the right, drag the element Nested Form right below this question.

 

Edit the element 

  • In the dropdown select the appropriate form
  • Enable Conditional Logic
  • Click on the Add Condition button to add the logic criteria
  • Select the reference you added to the previous question for example recordsrelease
  • Condition= Selected
  • Value= Yes

 

This conditional logic is setting the rules to determine if the nested form will be presented to the patient based on their response to the previous question by using conditional logic.

Add other Nested forms as needed and always be sure to Update to save your changes.
