What Are Unmatched Documents?
Unmatched documents are forms that have been completed in Flex but have not been successfully associated with a specific patient’s account.
Unmatched documents can occur due to:
- Incorrect or missing patient identifiers (e.g., name misspellings)
- Forms being linked on your website
- Forms submitted for a patient that is not in your Open Dental yet
Having unmatched documents in your system means that the information provided on the form is not imported into the patient's chart, which can lead to incomplete or inaccurate patient records.
How Do I Match Unmatched Documents?
Flex will alert you with a flashing '!' once you log in if you have any unmatched documents, and the icon will show normally when there are none seen below.

If your icon is flashing, simply click the icon and you will be prompted with a list of all your unmatched documents from the drop down, with a few options:
- Patient search
- Delete form
- View form
- Move form to patient
Please be aware, once a form is moved to a patient that forms data will sync to the patient it was moved to. If you incorrectly move a form to the wrong patient, you will need to manually move the form and update all information in the practice management system by hand.
Luckily, Flex can help you verify what patient you select to move to by showing not only the name of the patient but also the patient's patnum, or ID #:
If you are unsure who the form was filled out by utilize the 'view selected form' button and a .pdf version of the form will populate and you then have the ability to verify the information and the patient in question.
Once you have selected and verified you have the right patient, simply click the 'move form to patient' button and Flex will make sure you want to proceed:
Once you select 'got it' a successful prompt will appear and your form will now be attached in the patients account in your practice management system, the form will also be removed from the 'unmatched document' section in Flex.
